Behavioral Health Aide Certification

 

There will be three different certification applications, first is for the Prior Practice Certification (which is for grandparenting), the second is for the Initial Certification, and the third is for the Change in Level or Renewal of Certification.  

 

The Community Health Aide Program Certification Board (CHAPCB) is accepting electronic copies (PDF) or faxed certification applications but they must be fully completed prior to being submitted.  You may also mail the certification applications to the CHAPCB mailing address, their contact information is on each application.
