Hepatitis B Adult (age 20 & older) Vaccine Guidelines:
Before giving vaccine, screen all patients (including employees) not previously documented as immune for Hepatitis B (anti-HBc+ and/or ant-HBs+).
Vaccine is needed if the patient has:
1 - One or more of these Risk Criteria:
- - individuals with a chronic liver disease (including Hepatitis B and C carriers)
- - men who have sex with men
- - persons who use street drugs
- - persons who receive clotting factor
2 - If Hepatitis B vaccine series is started, but not complete:
- - finish off series no matter the timing
- - give only missing doses
- - the 3rd dose 5 months after the 2nd
3 - A negative (non-reactive) screen for anti-HBc & anti-HBs:
- Meaning that no Hep B core antibody is detected so no immunity by disease exists. If patient has never had the hepatitis B vaccine then:
- - Give 1st dose now, advise to complete series
- - 2nd dose in a month
- - 3rd (final) dose 5 months from 2nd
Checking patient's titers for response to vaccine is not recommended. There is no recommended booster at this time.
Post screen for an employee is required.
Remember, Hepatitis B vaccine is NOT needed in patients who meet any of these 3 parameters:
- - completed 3 doses of hepatitis B vaccine
- - are hepatitis B carriers
- - are immune by disease
- Note: Hepatitis B Vaccine for children ages 0-18 is provided by the State of Alaska
